1) download and fill out the “New Patient Form” PDF (2 pages).

2) bring a list of your medications

3) bring any insurance information you have,
e.g. your insurance card or brochure

4) if you are transferring from another dental office you can authorize their latest X-rays to be sent to us. Please download PDF (one page “Record of Release”).
